Scheduling Your Appointment
When calling for a new patient appointment, Allergy & Asthma
Consultants' receptionists PRE-REGISTER all new patients, so please be prepared to
provide the following patient's information:
- Reason for Appointment
- Full Name
- Home Address
- Phone numbers (Home and Alternate)
- Date of Birth
- Specific insurance information, including: Insured Party's Name, Date of Birth, Relationship to patient,
Social Security #'s, Group #'s, Claims Address and Phone #'s
- Referring physician, if applicable.
Without ALL of this information, we may not be able to schedule the appointment.
Each of these pieces of information is kept CONFIDENTIAL and
is essential to assure that your claims can be processed as cleanly and quickly as possible. This will
help to maximize your benefits for our services. This Pre-enrollment information will also help to make sure
that you are seen by the doctor in a timely manner and not waiting due to a delay in gathering this written
information after you arrive.
An initial appointment with testing can take approximately 2 hours, so please plan accordingly.
We make every attempt to supply you with the necessary information needed for your appointment. If lead
time permits, we will mail you an enrollment packet for you to complete prior to your appointment or you can
download them here for your convenience. The packet includes a Patient Enrollment Form with a
Health Questionnaire on the back, the HIPAA Authorization Form and our Welcome Letter to you. The Welcome
Letter contains instructions that you will need to follow prior to your appointment.
Our doctors will make every effort to perform any medically necessary testing during your initial visit.
This will save you time and money from having to schedule a separate appointment just for testing at a later
date.
The cost for allergy testing can vary greatly from patient to patient and insurance to insurance. Please
feel free to ask for general pricing guidelines.
What to Bring to Your
Appointment?
Remember: No ANTIHISTAMINES 48 to 72 Hours prior to your
Appointment !!!
- Your most recent insurance card(s)
- Physician referral forms, if required by your insurance
- List of prescriptions and/or over-the-counter medication(s), including dose and frequency
- Information about your medical and surgical history
- Recent X-ray, CAT Scan or MRI reports or relevant medical records
- Completed PATIENT PACKET (Patient Enrollment / Questionnaire and HIPAA Form) you may have received in the mail from
us
- Names, Addresses and Phone numbers of all relevant Physicians who have provided care for these
symptoms.
- Please arrive for your appointment 15 minutes prior to your scheduled time so that we may complete
setting up your chart, make copies of your insurance cards, etc.
- If you would like to have your related medical records sent to our office, please print our Medical Records Release Form.
PATIENT PACKET (Patient Enrollment / Questionnaire and HIPAA Form)
Insurance and Payment Information
Allergy & Asthma Consultants, Ltd., is a provider for Medicare and most major insurance
plans, and provides insurance billing. You are responsible for charges that are not covered by your
insurance, at the time of your appointment. If you have agreed to begin Immunotherapy, you will need to
complete and sign our Vial Authorization Form, BEFORE we can make your injection materials. Please click on

Your insurance company may require a co-payment at the time of your appointment. When necessary, our staff
will work with patients to set up a payment plan.
